Upon multiple viewings of the trailer, one particular moment captured my attention: at the 51-second mark, we see a hand, bloodied and dangling from a bed. In the background, a bald man wearing a blue shirt can be seen—matching Luther’s appearance in other scenes of the trailer. Additionally, the presence of medical monitors nearby strongly suggests a critical situation.
This detail is significant given Luther’s character arc. Having gone into hiding at the end of Dead Reckoning to aid in the fight against The Entity, it raises the question of who he is sheltering. While Ethan Hunt is a likely candidate, he appears in good health while greeting Luther later in the trailer. Thus, if Luther is indeed in hiding and not captured, the individual he’s with must be someone he deeply trusts.

Should the individual in question not be Ethan or Paris, my hypothesis is that it could very well be Ilsa Faust. Other characters, such as Grace, Benji, Brandt, and Julia, could theoretically fit into this scenario, but none of them currently display signs of being in danger or necessitating shelter with Luther. Bringing any of them into hiding seems unlikely without risking exposure of Luther’s concealed location.
The injured figure with Luther feels like it could be Ilsa Faust, especially considering the circumstances surrounding her previous demise. In Dead Reckoning, her apparent death was abrupt—seemingly the result of a fatal stabbing by Gabriel. However, the film does not provide closure regarding her body after the incident. Instead, the narrative shifts immediately to Ethan coordinating the subsequent heist.
I posit that Luther’s hurried departure could indicate an ongoing mission to safeguard Ilsa, who might have survived. The blood on the portrayed hand could trace back to Gabriel’s assault, enhancing the stakes of Luther’s storyline in The Final Reckoning. He could be actively working not just against The Entity, but also to ensure Ilsa’s recovery, thereby intertwining their fates.

Throughout Dead Reckoning, Ethan and the IMF face a constant battle to outmaneuver The Entity’s schemes. The AI demonstrates its predictive capabilities, declaring that either Ilsa or Grace is destined to die at a pivotal moment. This propels Ethan’s team into a frantic race to reclaim control of the situation. Thus, it is entirely feasible that Ethan and Luther orchestrated a ruse surrounding Ilsa’s death to mislead The Entity into believing they had succeeded in its plans.
This potential narrative twist would hint at Ethan’s growing strategic mastery over The Entity, showcasing his ability to maintain humanity and empathy in stark contrast to the AI’s cold calculations. Their efforts to preserve Ilsa’s life would emphasize Ethan’s character depth and determination to protect those he cares about.
It is worth noting that this scenario does not inherently contradict one of Luther’s parting lines in Dead Reckoning. While he implies that Ilsa’s death is a focal point of The Entity’s manipulation, this could also refer to Marie, an earlier loss grating against Ethan’s conscience. Ilsa’s narrow escape from death could serve as a fresh source of motivation for Ethan’s vendetta against Gabriel, whether or not she actually died.

Given her statements, it seems improbable that her return would align with the trajectory the character is currently on.
